In-person visit: When you have arrived to the clinic, please check in with our front staff

Telehealth visit:

  1. Prior to your scheduled appointment time, please make sure all consents and insurance information is updated. 

 2. Pay any copay or balance through the Simple Practice app. 

 3. Please note, your child must be present at his/her visit, and a current height and weight is required
