PREPARE FOR FIRST APPOINTMENT
Parking is available in the spots marked “Visitor” which are near the two main entrances on the east and west sides of the building.
Please arrive 15 minutes prior to your appointment time to complete paperwork
Please bring your insurance card, your current medications, a list of your previous medications, and copies of any previous neuropsychological testing or diagnostics
